Every two years, we ask our practices to fill out a survey from Blue Cross Blue Shield of Michigan evaluating our Coordinating Center. The survey was sent to all MOQC practice managers and clinical champions earlier this spring. The response rate this year was 31% (46/150). Below please find the action items to which we have committed, based on what we heard from the survey.

| - Consistently include physician champions on the regular communication that is already occurring with administrators and abstractors regarding data collection, performance, and quality improvement efforts
- Contact each practice at least once/year to schedule a connection visit for the purpose of reviewing performance and supporting improvement
