​Social Determinants (Drivers) of Health:

MOQC has partnered with the Patient Advocate Foundation, in collaboration with the Gilead Foundation, to develop a framework for assessing the social and financial factors affecting patients’ health. This partnership aims to create a support system that integrates social and financial support into the care delivery process. The goal is to build trust and meet patients’ needs while supporting the goals of medical practices. Three MOQC practices, MyMichigan Sault, Bronson Cancer Center, and West Michigan Cancer Center, will participate in this initiative. MOQC will review existing processes, identify gaps, and create an action plan to address them.

Financial Navigation Training:

Through a generous grant from the Michigan Department of Health and Human Services, MOQC is partnering with Dan Sherman, a financial navigation expert, to offer three virtual training sessions for MOQC practice members at no cost. These sessions will educate participants on Medicare, Medicaid, and Marketplace coverage options, as well as ways to navigate insurance assistance programs, free medication resources, and basic needs support for cancer patients. All seven MOQC regions will be represented in these sessions. At the end of the training, we will hold a virtual meeting to discuss implementation, address any gaps, and allow trainees to collaborate with MOQC to create region-specific resources. The sessions will take place in March, April, and May. Due to high demand, all sessions are fully booked.
