MOQC Abstractors have at least 60 different logins and passwords to abstract for all the practices that have MOQC perform abstraction. This is for both electronic medical records systems and for QOPI login.

If a patient is noted to have a family history of cancer, ages of ALL relatives (1st and 2nd degree) at time of diagnosis must be documented or documented as unknown to be considered complete and meet the family history measure.
