How To Get Started with Participation:
In order for MOQC to support practice and Michigan quality improvement efforts, a Data Use Agreement (DUA) between the practice and MOQC is necessary. Only blinded data will be used in benchmarking reports. Practice-specific data will not be shared with BCBSM. Completion of the Practice Profile ensures we have current information about your practice should we need to contact you.
For your convenience, we have included the DUA, Practice Profile and Fax Cover sheet for use in returning the documents to MOQC.
